Comment remplir un formulaire PDF | Filling-in and submitting a PDF form

Les formulaires PDF de feuille verte et de demande de badge peuvent être remplis directement dans un navigateur muni d'un plug-in Acrobat Reader :

Remplissez le formulaire et cliquez ensuite sur le bouton ENVOYER pour l'expédier par mail. Si le bouton est inopérant, il faut ouvrir le fichier PDF et le remplir directement dans Acrobat Reader. Vous avez également la possibilité de l'enregistrer dans votre ordinateur, l'imprimer, le remplir à la main et de le joindre par mail à l'adresse suivante : ohp.badges@oamp.fr

For best results, you should fill in the PDF form with a web browser and an Acrobat Reader plug-in :

Clicking on the ENVOYER/SEND button should normally submit the form as an attachment through your regular e-mail client. However, this may not be possible in some situations, since it depends on your specific computer platform, browser and plug-in.

In case you are not able to submit the form in this manner, please open it directly with Acrobat Reader, proceed to fill it normally and then send it. As a last resort, print out the PDF file, fill it by hand and post it by regular mail, fax it or e-mail a scan of it.
